I managed to do this, however I did not use multiple submit buttons
I used radio buttons instead and used a perl script to format the output.
You can see it under the Completed projects section of
URL=http://www.med.umich.edu/~jauderho/. The scripts I used are also there
so you can copy it off of that. You can easily modify it to a checkbox type
where you can choose multiple people to send the message to.
